How to make an appointment 

If you would like to have a session, you can do so in a number of ways: by  phone: 24 hours a day (450) 686-2440 and leave a message with your name and number where you can be reached.  

Leave me a voice-mail message and I shall get back to you as soon as possible. If your prefer e-mail send to Howard Gontovnick.  Using this format, the same instructions apply with regard to getting back to you and confirming a time and date
. 

If you prefer, you can write a hand-written letter and send it by (a) regular mail or  (b) drop the letter in person at my office. I do have a professional office which is located at:

Fees & Method of payment

The cost per session (minimum 1 hour) is pre-established prior to the actual time and includes the usual applicable taxes in Canada (GST and PST). The amount is fixed and doesn't change throughout the course of the treatment program.

There are ONLY two methods of payment:

(a) Cash. 

(b) Personal cheque: (payable)   Howard Gontovnick.
  

Each session is based on a reasonable fee for a complete 60 minute session. 

Upon the conclusion of each session,  a professional receipt is issued. This document can serve either as income tax references or for insurance purposes.

I should like to mention, that I am very concerned when it comes to confidentiality and respecting the privacy of each individual. Keep in mind, that most insurance companies require the therapist to report private intimate details about clients' lives and background to their situation. Therefore, I would caution you when it comes to submitting your personal information of this nature. Even though the fee is covered by your insurance company, the security and storage of your information in these matters should be limited and well protected.

All information is confidential and will remain so unless otherwise authorized (in writing only).
